These Terms & Conditions govern your participation in the orthopedic research program administered by John G. Lane MD Inc. (“we,” “us,” or “our”).

Research Participation

By enrolling in our research program, you agree to complete follow-up surveys at scheduled post-intervention timepoints. Participation is voluntary and you may withdraw at any time without penalty.

SMS Text Messaging Program

Program Name: John G. Lane MD Inc. Orthopedic Research Follow-Up Program

SMS Consent

By providing your mobile phone number on our intake form and checking the SMS consent checkbox, you are directly consenting to receive automated text message survey reminders from John G. Lane MD Inc. specifically for our orthopedic research follow-up program. This consent is specific to this program only.

Your consent to receive SMS messages is separate from any other agreement and is not required to participate in the research study. You may withdraw SMS consent at any time by replying STOP.

We do not share, sell, or provide your mobile phone number or messaging consent data to third parties or affiliates for marketing or promotional purposes. Mobile information and messaging opt-in consent will not be shared with any third parties for marketing or promotional purposes under any circumstances. This is not an affiliate marketing or third-party lead generation program.

SMS Terms of Service

By providing your mobile phone number and opting in to SMS communications, you agree to the following terms:

• Program description: John G. Lane MD Inc. sends automated SMS text message reminders containing secure links to follow-up research surveys.
• Message frequency: You may receive up to 3 reminder messages at each study follow-up time point: before surgery (preop), 2 weeks, 6 weeks, 3 months, and 6 months after surgery, and annually thereafter for up to 5 years. This is a maximum of approximately 30 messages over the course of your participation.
• Message and data rates may apply. Check with your wireless carrier for details.
• Opt-out: Reply STOP to any message to unsubscribe from SMS communications at any time. You will receive a confirmation message and no further messages will be sent.
• Opt-in: Reply START or UNSTOP to resubscribe to SMS communications.
• Help: Reply HELP for assistance, or contact us at 858-292-1433 or Office@johnglanemd.com.

Data Use

Survey data collected through our REDCap platform (hosted by UCSD Health Sciences) is used solely for orthopedic research purposes. Data is stored in compliance with HIPAA regulations. All data sharing is limited to research purposes only and is never used for marketing or promotional activities.

Limitation of Liability

John G. Lane MD Inc. is not liable for any delays or failures in SMS delivery. Message delivery is subject to your wireless carrier’s network availability. Carriers are not liable for delayed or undelivered messages.

Privacy

Your use of this messaging program is also governed by our Privacy Policy.
